Toxicology - Review and Release (read‑only with RX Verification)

Use the Toxicology Review and Release form to verify drug screen/confirmation results and their RX Verification interpretations before finalizing. This screen is read‑only—no values can be entered or changed here. To make edits, click Edit Result to open Toxicology Result Entry, apply changes, Save, and return to Review and Release.

Key differences vs. Result Entry

  • Read‑only: Results, comments, and medications cannot be edited here.
  • Edit Result: Jumps to Toxicology Result Entry to make changes.
  • RX Verification visible: Per‑analyte and overall Consistent/Inconsistent interpretations are displayed for final review.

What you can do

  • Load worklists by date range, client/site, test/panel, status (e.g., Pending Release).
  • Review per‑analyte results (screen/confirm), cutoff flags, specimen validity (e.g., creatinine, pH), and RX Verification interpretations.
  • Preview reports (single or batch) without triggering distribution.
  • Release All or Release Selected for partial releases.
  • Mark as Reviewed (if enabled) to document verification without releasing.

📘 Instructions: Review and release toxicology results

  1. Open Toxicology → Review and Release.
  2. Apply filters (date, client/site, test/panel, status and click Find.
  3. Review each order/test:
    • Results and units; Screen vs. Confirm status
    • Specimen validity/adulteration flags
    • Reported medications and mapping (parent/metabolites/classes)
    • RX Verification per analyte and overall interpretation
  4. Need to change anything?
    • Click Edit Result to open Toxicology Result Entry, make updates (results, comments, medications), Save, then return.
  5. Optional: Click Preview to verify report content/format (Preview does not distribute).
  6. Release:
    • Select specific tests for partial release, or choose Release All.
    • Complete any prompts.
  7. Verify delivery:
    • Check delivery logs/ACKs (fax/portal/HL7/email) and requeue failures if needed.

What to verify before release (tox checklist)

  • Confirmatory status: Presumptive positives reflexed and, if appropriate, confirmed.
  • Consistency logic: RX Verification aligns with reported meds and expected metabolites.
  • Inconsistent cases: Rationale/comment documented (e.g., timing, cutoffs).
  • Specimen validity: No unresolved adulteration/substitution; if invalid, ensure interpretation reflects “Unable to evaluate” or per policy.
  • Distribution readiness: Hold results until confirmations are back, if required by your SOP.

Distribution behavior

  • Preview: No Report Rules run, and results do not appear on the portal.
  • Release: Triggers Report Rules and portal visibility.
  • Partial release: Sends an interim report for the selected items; unreleased components can be finalized later.

Best practices

  • Require a concise note for any Inconsistent interpretation before release.
  • Keep parent–metabolite mappings and cutoff tables current; route complex cases to a senior reviewer.
  • Hold final distribution until confirmatory testing resolves presumptive positives (per policy).
  • For corrected results after release, use the correction workflow to generate a Corrected report and re‑distribute.

Troubleshooting

  • Can’t edit: Expected here. Click Edit Result to modify in Toxicology Result Entry.
  • Unable to Release: Check permissions, order holds, or missing required documentation (e.g., inconsistency rationale).
  • RX Verification looks wrong: Open Edit Result to verify reported meds, mapping (parent/metabolites/class), and detection window; save and return.
  • Portal didn’t update: Ensure items were Released (Final/Corrected) and check delivery logs; requeue if needed.
