Cancellation Reason Master File
Cancellation Reasons — Purpose and workflow
Use Cancellation Reasons to standardize why lab orders (including standing orders) are canceled. When an order is canceled, the selected reason is displayed on the report with a “Canceled” watermark so clinicians understand exactly why the test did not proceed.
- Workflow rules: Based on the chosen reason, the system can prompt or automatically create a replacement order to prevent missed results. Configure these Cancellation Rules in the Client Master File.
- Audit trail: Store the reason, user, timestamp, and optional comments for clear traceability and compliance.
- Standing orders: Apply the same reasons to canceled standing orders to maintain a consistent historical record.
Examples of common reasons include: patient refusal, duplicate order, wrong patient/specimen, insufficient quantity (QNS), wrong container, specimen stability exceeded, test not indicated, client request, instrument/processing failure, eligibility/authorization issues, or specimen not received.
