The Blank Requisition feature allows the LIS to generate blank lab requisitions that can be distributed to clients who prefer to complete requisition forms by hand.
This feature is especially useful for clients who do not want pre-filled electronic requisitions and instead need printed forms with a customized test menu based on their ordering patterns.
The main advantage of this feature is the ability to create a customized test menu for each client based on their specific needs and preferences.
For example:
Pain clinics often order toxicology testing for drug screening and confirmation.
These clients may require custom panels designed specifically for their clinic workflows.
Additional benefits include:
The lab can print blank requisitions in-house, reducing the need to order large volumes of pre-printed forms.
This helps avoid:
storage issues
unnecessary bulk ordering
waste caused by changes to the test menu that make previously printed requisitions obsolete
The requisition form can be customized not only for the test menu, but also for the overall design, including:
field types
field sizes
colors
layout and formatting
The system includes a default blank requisition template out of the box.
If you want to design your own template, go to:
Tools > Reports > Custom Reports > Report Designer
From there, you can create a new Blank Requisition template.
If you do not need a custom design, you can use the default template and simply configure the test menu for each client.
There are two ways to configure this feature, one of which is through the Client Master File.
To configure it from the Client Master File:
Open Client Master File
Select the desired client
Click More Options
Select Blank Requisition Settings
Once the Blank Requisition Settings form opens, complete the following steps:
Choose how many carbon copies should be printed.
The system will generate:
one original, plus
the selected number of carbon copies
All copies will use the same barcode number.
This option is useful for labs that want to leave one copy at the client’s location while retaining the original or additional copies for internal use.
Choose the tests that should appear on the blank requisition for that client.
This allows each client to have a test menu tailored to the services they typically order.
Organize the test menu by dragging and dropping the tests into the desired order.
This helps present the most commonly ordered tests in a more convenient layout for the client.
If the client uses panels, you can decide whether or not to display the tests associated with those panels.
Options include:
Yes: display the tests included in the panel
No: do not display the associated tests
You can also define how many levels of the test tree should be shown:
For standard panels, only one level is available
For super panels, you can select up to two levels
If another client already has a similar test menu configured, you can save time by copying that setup.
To do this:
Click Copy From
Select the client whose menu you want to copy
This is especially helpful when multiple clients use similar testing profiles.
After completing all required settings, click Save.
The client is now ready for blank requisition printing.
